Surgical treatment for urinary stones

Urinary stone surgery is indicated when conservative management—such as increased fluid intake, medical expulsive therapy, and analgesia—fails or is inappropriate due to stone size, location, composition, or associated complications. Common surgical interventions include extracorporeal shock wave lithotripsy (ESWL), ureteroscopy with laser lithotripsy, percutaneous nephrolithotomy (PCNL), and, rarely, open or laparoscopic stone removal. ESWL uses focused acoustic pulses to fragment stones in the kidney or upper ureter but is less effective for large, hard (e.g., cystine or calcium oxalate monohydrate), or radiolucent stones. Ureteroscopy is the preferred minimally invasive option for mid- to distal ureteral stones and many renal stones; it allows direct visualization and fragmentation using holmium:YAG laser energy, with immediate stone clearance. PCNL is reserved for larger renal stones (>2 cm), staghorn calculi, or stones refractory to other modalities; it involves a percutaneous tract into the renal collecting system under fluoroscopic or ultrasound guidance, followed by endoscopic stone fragmentation and extraction. Patient selection considers comorbidities, anatomy (e.g., obesity, spinal deformity), stone burden, renal function, and infection status—particularly urgent intervention for obstructing stones with concurrent urinary tract infection (obstructive pyelonephritis), which constitutes a urologic emergency requiring prompt drainage (e.g., nephrostomy tube or ureteral stent) prior to definitive stone treatment.
