Dizziness when turning over in bed
Experiencing dizziness when rolling over in bed is a common symptom often associated with benign paroxysmal positional vertigo (BPPV). This condition arises when tiny calcium carbonate crystals—called otoconia or “ear stones”—become dislodged from their normal location in the utricle of the inner ear and migrate into one of the semicircular canals, most frequently the posterior canal. When you change head position—such as turning to the side while lying down or sitting up—the displaced crystals shift within the fluid-filled canal, sending false signals to the brain about head movement. This mismatch between actual motion and vestibular input triggers brief but intense episodes of rotational vertigo, typically lasting less than one minute.
Other potential causes include vestibular neuritis, labyrinthitis, migraine-associated vertigo, or, less commonly, central nervous system disorders such as cerebellar or brainstem pathology—especially if dizziness is accompanied by neurological symptoms like double vision, slurred speech, limb weakness, or gait instability. Certain medications, dehydration, orthostatic hypotension, or anxiety-related hyperventilation may also contribute to positional dizziness, though these usually lack the characteristic spinning sensation and strict positional dependence seen in BPPV.
If this symptom is new, recurrent, or significantly affecting daily function, it’s important to consult a healthcare provider for proper evaluation. Diagnosis often involves clinical maneuvers such as the Dix–Hallpike test, which can provoke characteristic nystagmus and vertigo to confirm BPPV and identify the affected canal. Treatment is highly effective and typically includes canalith repositioning procedures—most notably the Epley maneuver—for posterior canal BPPV. These non-invasive, office-based maneuvers guide the displaced otoconia back to a location where they no longer cause symptoms. In many cases, patients experience significant improvement after just one or two sessions.
