While occasional crusting may occur after minor trauma or during dry winter months, chronic or recurrent nasal crusts frequently signal an underlying issue requiring clinical evaluation.
The most frequent cause is mucosal dryness, often due to environmental factors such as low humidity, prolonged exposure to heated indoor air, or excessive use of nasal decongestant sprays (e.g., oxymetazoline), which can induce rebound congestion and mucosal atrophy. Chronic rhinitis—whether allergic, nonallergic, or infectious—may also disrupt normal mucociliary clearance and epithelial integrity, predisposing to microtrauma and subsequent crusting.
More concerning etiologies include inflammatory conditions like granulomatosis with polyangiitis (GPA), sarcoidosis, or relapsing polychondritis, all of which can manifest with nasal ulceration, septal perforation, and persistent crusting. Infectious causes—such as syphilis, tuberculosis, or fungal sinusitis—must be considered in patients with systemic symptoms, immunocompromise, or failure to respond to conservative management.
Other contributors include habitual nose-picking (digital trauma), chronic nasal cannula use, or iatrogenic injury following endoscopic sinus surgery or septoplasty. Rarely, squamous cell carcinoma or other sinonasal malignancies present with unilateral, progressive crusting, epistaxis, and nasal obstruction.
Evaluation should begin with thorough history—including duration, laterality, associated symptoms (e.g., anosmia, facial pain, systemic complaints)—and anterior rhinoscopy. Nasal endoscopy is essential to assess mucosal appearance, identify ulceration or perforation, and guide targeted biopsy if suspicious lesions are present. Imaging (e.g., contrast-enhanced CT or MRI) may be warranted for suspected neoplastic or invasive inflammatory disease.
Initial management focuses on humidification, saline irrigation, and emollient ointments (e.g., petrolatum-based nasal gels). Avoidance of nasal decongestants and irritants is critical. Persistent or refractory cases require referral to otolaryngology for comprehensive assessment and potential histopathologic or microbiologic workup.
