These symptoms frequently co-occur and point to a disruption in the scalp’s natural homeostasis—most commonly seborrheic dermatitis, a chronic inflammatory condition driven by an abnormal immune response to the commensal yeast *Malassezia*.
Seborrheic dermatitis affects up to 5% of the general population and is particularly prevalent in individuals with certain systemic conditions—including Parkinson’s disease, HIV infection, and depression—as well as those experiencing significant stress or seasonal changes. The condition thrives in areas rich in sebaceous glands: the scalp, eyebrows, nasolabial folds, and upper chest. On the scalp, it manifests as greasy, yellowish scales adherent to erythematous (red), inflamed skin—distinct from the dry, flaky plaques seen in psoriasis or the fine, non-inflammatory shedding of simple pityriasis capitis (common dandruff).
While *Malassezia* species are part of the normal scalp microbiome, overgrowth—often fueled by excess sebum production—triggers keratinocyte activation and local inflammation. This leads to accelerated epidermal turnover, resulting in visible scaling, pruritus (itching), and sometimes secondary bacterial colonization. Importantly, frequent shampooing alone does not resolve the underlying inflammation; in fact, harsh surfactants may further disrupt the stratum corneum barrier and exacerbate irritation.
First-line management includes antifungal shampoos containing ketoconazole, ciclopirox, or zinc pyrithione, used two to three times weekly for at least four weeks. For moderate-to-severe cases, low-potency topical corticosteroids (e.g., hydrocortisone 1%) or calcineurin inhibitors (e.g., tacrolimus 0.1%) may be added short-term to control inflammation. Long-term maintenance therapy—such as weekly antifungal use—is often necessary to prevent relapse. Patients should also avoid occlusive hair products and minimize mechanical trauma (e.g., aggressive brushing or heat styling) that can worsen follicular irritation.
If symptoms persist despite appropriate treatment, clinicians should consider differential diagnoses—including tinea capitis, contact dermatitis, psoriasis, or even early-stage cutaneous lymphoma—and pursue targeted evaluation, such as scalp biopsy or fungal culture, when indicated.
