Primary amenorrhea refers to the absence of menarche by age 15 in individuals with normal growth and secondary sexual development—or by age 13 in those lacking any signs of puberty. Secondary amenorrhea is defined as the absence of menstruation for at least three consecutive cycles in individuals who previously experienced regular menses, or for six months in those with irregular cycles—excluding pregnancy, lactation, or menopause as causes.
Beyond this fundamental dichotomy, clinicians further categorize amenorrhea based on underlying pathophysiology. Hypothalamic amenorrhea arises from disruptions in gonadotropin-releasing hormone (GnRH) pulsatility—often linked to stress, excessive exercise, or low energy availability. Pituitary causes include hyperprolactinemia, prolactinomas, or Sheehan syndrome. Ovarian etiologies encompass premature ovarian insufficiency, genetic conditions such as Turner syndrome, and chemotherapy-induced follicular depletion. Structural abnormalities—such as Müllerian agenesis, intrauterine adhesions (Asherman syndrome), or cervical stenosis—constitute uterine or outflow tract causes. Finally, endocrine disorders like uncontrolled thyroid disease, non-classical congenital adrenal hyperplasia, or Cushing syndrome may also manifest as amenorrhea.
Accurate classification guides targeted diagnostic evaluation—including serum hormone assays (FSH, LH, prolactin, TSH, estradiol), pelvic imaging, and, when indicated, karyotyping or MRI—and informs evidence-based management strategies ranging from lifestyle modification and hormonal therapy to surgical intervention.
