What does a color Doppler ultrasound of the liver, gallbladder, spleen, and pancreas evaluate?
Abdominal ultrasound—specifically, a focused evaluation of the liver, gallbladder, spleen, and pancreas—is a noninvasive, radiation-free imaging modality used to assess the size, shape, contour, echotexture, and internal architecture of these organs. It evaluates for structural abnormalities such as hepatic steatosis (fatty infiltration), cirrhosis, focal lesions (e.g., cysts, hemangiomas, or metastases), gallstones, biliary sludge, gallbladder wall thickening or polyps, splenomegaly or focal splenic lesions, and pancreatic enlargement, calcifications, cysts, or mass lesions. The exam also assesses the patency of major bile ducts (common bile duct, intrahepatic ducts) and detects signs of biliary obstruction, such as ductal dilation. While highly sensitive for detecting gallstones and many liver lesions, ultrasound has limitations—including reduced sensitivity for small pancreatic lesions or early-stage hepatocellular carcinoma—and may be limited by patient factors like obesity or excessive bowel gas. When findings are equivocal or further characterization is needed, complementary imaging—such as contrast-enhanced CT, MRI with MRCP, or endoscopic ultrasound—may be recommended.
