Sweating is a critical thermoregulatory function mediated by eccrine glands, and its impairment can pose serious health risks, including heat exhaustion or heat stroke.
Common contributors include autonomic nervous system dysfunction, such as in diabetic neuropathy or Parkinson’s disease, where nerve signaling to sweat glands is disrupted. Certain medications—particularly anticholinergics (e.g., antihistamines, tricyclic antidepressants, and some antipsychotics)—can inhibit acetylcholine release, thereby suppressing sweat production. Skin disorders like scleroderma, psoriasis, or severe burns may damage or replace eccrine glands, leading to localized or generalized anhidrosis.
Genetic conditions, such as hypohidrotic ectodermal dysplasia, result from mutations affecting ectoderm-derived structures—including sweat glands—and typically present from childhood with sparse hair, dental anomalies, and impaired sweating. Autoimmune diseases like Sjögren’s syndrome or systemic lupus erythematosus may also involve glandular infiltration or inflammation that compromises sweat gland function.
Importantly, isolated absence of sweating in women is rarely idiopathic; it warrants thorough clinical evaluation. Assessment should include detailed history (e.g., onset, distribution, associated symptoms), physical examination, thermoregulatory sweat testing, and targeted investigations—such as nerve conduction studies, skin biopsy, or autoimmune serology—depending on suspected etiology. Early diagnosis and management are essential not only for symptom relief but, more critically, for preventing life-threatening hyperthermia.
