Chronic periodontitis is a prevalent inflammatory disease affecting the supporting structures of the teeth—including the gingiva, periodontal ligament, and alveolar bone—and is a leading cause of tooth loss in adults worldwide. While it can occur across all age groups, certain populations face significantly elevated risk due to a combination of biological, behavioral, and socioeconomic factors.
Individuals aged 35 years and older represent the highest-risk demographic, with prevalence increasing steadily with age. This reflects cumulative exposure to bacterial biofilm, prolonged inflammation, and age-related declines in immune surveillance and tissue repair capacity.
Smokers constitute another high-risk group: tobacco use impairs neutrophil function, reduces gingival blood flow, and suppresses collagen synthesis—thereby accelerating attachment loss and bone resorption. Smokers are up to six times more likely to develop severe chronic periodontitis compared with nonsmokers.
Patients with uncontrolled type 2 diabetes mellitus are also at markedly increased risk. Hyperglycemia promotes advanced glycation end-product formation, exacerbates oxidative stress, and dysregulates cytokine production—creating a pro-inflammatory milieu that amplifies periodontal tissue destruction. The relationship is bidirectional: severe periodontitis can further impair glycemic control.
Additional high-risk cohorts include individuals with genetic polymorphisms in interleukin-1 (IL-1) genes, which predispose to exaggerated host inflammatory responses; those with long-standing untreated dental plaque and calculus accumulation; and patients with systemic conditions such as rheumatoid arthritis or cardiovascular disease—conditions sharing underlying inflammatory pathways with periodontitis.
Socioeconomic determinants—including limited access to dental care, low health literacy, and occupational stress—further compound risk, particularly among underserved populations. Early identification of these high-risk groups enables targeted prevention strategies, including personalized oral hygiene instruction, more frequent professional monitoring, and interdisciplinary management involving dentists, physicians, and public health professionals.
