Influenza A virus infection—commonly referred to as “H1N1 flu” or “swine flu”—is a highly contagious acute respiratory illness caused by influenza A viruses. These viruses belong to the Orthomyxoviridae family and are classified into subtypes based on two surface glycoproteins: hemagglutinin (H) and neuraminidase (N). The H1N1 subtype, which emerged globally in 2009 as a pandemic strain, remains one of the predominant seasonal influenza A variants circulating worldwide.
The virus spreads primarily through respiratory droplets generated when infected individuals cough, sneeze, or talk. It can also transmit via contact with contaminated surfaces followed by self-inoculation of the eyes, nose, or mouth. Incubation typically lasts one to four days, and infectiousness peaks within the first three days of symptom onset—though viral shedding may persist longer in immunocompromised individuals or young children.
Clinical presentation ranges from mild upper respiratory symptoms—including fever, chills, sore throat, rhinorrhea, myalgia, headache, and fatigue—to more severe manifestations such as pneumonia, acute respiratory distress syndrome (ARDS), myocarditis, or secondary bacterial infections. High-risk populations—including children under five years, adults over 65, pregnant individuals, and those with chronic cardiopulmonary, metabolic, or immunosuppressive conditions—are at significantly increased risk for complications and hospitalization.
Diagnosis is confirmed via molecular testing, most commonly real-time reverse transcription polymerase chain reaction (rRT-PCR) performed on nasopharyngeal or oropharyngeal swabs. Rapid antigen tests offer quicker results but have lower sensitivity, particularly later in the illness course. Antiviral therapy—such as oseltamivir, zanamivir, or baloxavir—is most effective when initiated within 48 hours of symptom onset and is strongly recommended for high-risk patients or those with progressive disease.
Annual vaccination remains the cornerstone of prevention. Current quadrivalent influenza vaccines include an H1N1 strain component, updated annually based on global surveillance data from the World Health Organization’s Global Influenza Surveillance and Response System (GISRS). Non-pharmaceutical interventions—including hand hygiene, respiratory etiquette, and appropriate isolation during illness—further reduce transmission risk in both community and healthcare settings.
