Retropertitoneal hematoma refers to the accumulation of blood in the retroperitoneal space—the anatomical region located behind the peritoneum, the serous membrane that lines the abdominal cavity and covers most intra-abdominal organs. This space extends from the diaphragm superiorly to the pelvic inlet inferiorly and is bounded anteriorly by the posterior parietal peritoneum and posteriorly by the lumbar fascia and vertebral column.
Retroperitoneal hematomas most commonly arise from traumatic injury—such as blunt or penetrating abdominal or pelvic trauma—or iatrogenic causes, including complications from anticoagulant therapy, vascular interventions (e.g., femoral artery catheterization), or surgical procedures involving retroperitoneal structures like the kidneys, aorta, or iliac vessels. Less frequently, they may occur spontaneously in patients with coagulopathies, underlying vascular anomalies (e.g., aneurysms or arteriovenous malformations), or malignancies invading adjacent vasculature.
Clinical presentation varies widely depending on size, rate of bleeding, and associated injuries. Patients may present with nonspecific symptoms such as flank or abdominal pain, hypotension, tachycardia, or signs of shock. Physical examination often reveals minimal or absent abdominal tenderness despite significant hemorrhage, due to the compartmentalized nature of the retroperitoneum—which can accommodate large volumes of blood before manifesting overt clinical signs. Laboratory findings may include declining hemoglobin levels, though initial values can be deceptively normal in acute settings.
Diagnosis relies heavily on imaging. Contrast-enhanced computed tomography (CT) is the gold standard, offering high sensitivity for detecting hematoma location, extent, active extravasation, and potential sources of bleeding. Ultrasound may serve as an initial screening tool in unstable patients, while angiography remains both diagnostic and therapeutic when endovascular intervention is indicated.
Management is guided by hemodynamic stability, etiology, and bleeding severity. Hemodynamically stable patients with small, nonexpanding hematomas may undergo conservative management with close monitoring, reversal of anticoagulation if applicable, and serial imaging. In contrast, unstable patients or those with ongoing hemorrhage typically require urgent intervention—ranging from transfusion and resuscitation to emergent surgical exploration or endovascular embolization. Mortality remains substantial, particularly when diagnosis is delayed or when massive hemorrhage leads to multiorgan dysfunction.
