Uterine perforation is a rare but potentially serious complication that can occur during gynecologic procedures such as intrauterine device (IUD) insertion, endometrial biopsy, dilation and curettage (D&C), hysteroscopy, or even spontaneous abortion. It involves an unintended full-thickness tear through the uterine wall—either into the myometrium, peritoneal cavity, or adjacent structures like the bowel or bladder.
Management depends on several critical factors: the size and location of the perforation, the presence or absence of active bleeding or organ injury, the patient’s hemodynamic stability, and whether any instrumentation remains embedded in the uterine wall or surrounding tissues. In stable patients with small, asymptomatic perforations—particularly those identified incidentally during IUD placement—conservative management is often appropriate. This includes close clinical observation, serial vital signs, and pelvic ultrasound to confirm device position and rule out intraperitoneal migration or hematoma formation.
When surgical intervention is indicated—such as in cases of persistent pain, hemodynamic instability, suspected visceral injury, or retained foreign bodies—laparoscopy is typically the first-line approach. It allows for precise visualization, assessment of adjacent organs, and minimally invasive repair. Laparoscopic suture repair of the uterine defect is performed when feasible; however, if extensive tissue damage or contamination is present, conversion to laparotomy may be necessary. In rare instances involving severe hemorrhage or irreparable injury, hysterectomy may be required—but this remains an exceptional last resort.
Antibiotic prophylaxis is recommended when there is concern for bacterial contamination—especially if bowel or bladder injury is suspected—or when surgical repair is undertaken. Post-procedural follow-up should include counseling on signs of infection or delayed complications, such as chronic pelvic pain or adhesion-related infertility, and consideration of alternative contraceptive methods if an IUD was involved.
Prevention remains paramount: meticulous technique, real-time ultrasound guidance during high-risk procedures, and thorough pre-procedure assessment—including evaluation of uterine anatomy and risk factors such as prior cesarean delivery or uterine anomalies—can significantly reduce incidence. Early recognition and prompt, individualized intervention are essential to minimizing morbidity and preserving reproductive health.
