As global populations age, the prevalence of benign prostatic hyperplasia (BPH) continues to rise. Epidemiological data indicate that approximately 50% of men around age 60 are affected by BPH, with prevalence climbing to nearly 90% among those aged 80 and older. In response to this growing clinical burden, combination pharmacotherapy has emerged as a promising strategy—most notably, the recently approved fixed-dose combination capsule containing finasteride and tadalafil, marketed in China as Aitinglie.
Aitinglie represents the first domestically approved fixed-dose combination therapy for BPH in China, addressing a longstanding gap in treatment options. Each capsule delivers 5 mg of finasteride and 5 mg of tadalafil—a rationally designed dual-mechanism formulation targeting both the structural progression and symptomatic burden of BPH.
Finasteride, a selective type II 5α-reductase inhibitor, reduces intraprostatic dihydrotestosterone (DHT) concentrations by blocking the enzymatic conversion of testosterone to DHT. This leads to gradual prostate volume reduction and long-term disease modification—slowing progression and decreasing risk of acute urinary retention or surgical intervention. Tadalafil, a phosphodiesterase type 5 (PDE5) inhibitor, enhances smooth muscle relaxation in the bladder neck, prostate stroma, and urethra via increased intracellular cyclic guanosine monophosphate (cGMP). This results in rapid improvement of lower urinary tract symptoms (LUTS), including urinary hesitancy, weak stream, nocturia, and urgency.
Clinical evidence demonstrates that the finasteride–tadalafil combination produces superior symptom control compared with finasteride monotherapy. In pivotal trials, patients treated with the combination showed significantly greater reductions in International Prostate Symptom Score (IPSS) at both early (4-week) and sustained (24-week) timepoints. Importantly, tadalafil’s favorable effect on erectile function and ejaculatory parameters may also mitigate sexual adverse effects commonly associated with 5α-reductase inhibitors—enhancing overall tolerability and quality of life.
From a practical standpoint, the once-daily dosing regimen improves adherence and reduces prescribing and administration errors—critical advantages in an aging population managing multiple chronic conditions. However, optimal outcomes require more than pharmacotherapy alone. Lifestyle modifications play a vital supportive role: avoiding prolonged sitting or cycling, limiting evening fluid intake (especially caffeine and alcohol), maintaining regular bowel habits to prevent rectal pressure on the bladder neck, engaging in low-impact aerobic activity such as brisk walking or tai chi, and ensuring adequate thermal protection of the pelvic region.
While Aitinglie offers a mechanistically integrated approach—simultaneously addressing pathophysiology and symptomatology—it is not a universal solution. Treatment decisions must be individualized based on prostate size, symptom severity, urinary flow metrics, comorbidities, and patient preferences. Urodynamic evaluation, transrectal ultrasound, and post-void residual urine measurement remain essential components of comprehensive BPH management. As with all prescription therapies, initiation and ongoing use should occur under the supervision of a qualified urologist or primary care provider experienced in men’s health.
