When patients experience simultaneous toothache and ear pain, it’s often not due to a primary ear disorder—but rather a phenomenon known as referred pain. The trigeminal nerve (cranial nerve V), which innervates the teeth, jaw, and much of the face, shares overlapping neural pathways with the glossopharyngeal nerve (cranial nerve IX) and the upper cervical spinal nerves, both of which supply sensation to the external ear and middle ear structures. As a result, nociceptive signals from dental or periodontal pathology—such as irreversible pulpitis, acute apical periodontitis, or third molar impaction—can be misinterpreted by the central nervous system as originating in the ear.
This pattern is especially common with posterior teeth, particularly mandibular molars and premolars, given their proximity to the temporomandibular joint (TMJ) and the auriculotemporal branch of the trigeminal nerve. Inflammation or infection in these teeth may also trigger secondary muscle spasm in the lateral pterygoid or masseter muscles, further contributing to otalgia via myofascial referral. Less frequently, true otologic causes—such as acute otitis media or otitis externa—may coexist or mimic dental pain, underscoring the importance of thorough clinical evaluation.
A comprehensive assessment should include dental examination (percussion testing, thermal pulp testing, radiographic imaging), otoscopic evaluation, and TMJ assessment. Failure to recognize dental origin can lead to delayed treatment, progression to abscess formation, or unnecessary antibiotic prescriptions for presumed ear infection. Clinicians across disciplines—including dentists, primary care providers, and otolaryngologists—must maintain a high index of suspicion for odontogenic referral when evaluating unilateral ear pain without objective otoscopic findings.
