Marketed in China as Saiximei (Xiamen HaiBo Pharmaceutical), bempedoic acid is an oral, first-in-class ATP citrate lyase (ACL) inhibitor that reduces hepatic cholesterol synthesis upstream of HMG-CoA reductase.
Bempedoic acid is indicated as an adjunct to diet and maximally tolerated statin therapy for adults with heterozygous familial hypercholesterolemia or established atherosclerotic cardiovascular disease (ASCVD) who require additional LDL-C lowering. It is especially appropriate for patients who cannot tolerate higher-dose statins due to muscle-related adverse effects, given its distinct mechanism of action and lack of systemic exposure in skeletal muscle.
