This state may arise from a range of underlying causes, including polycythemia (elevated red blood cell mass), monoclonal gammopathies such as multiple myeloma or Waldenström macroglobulinemia, severe dehydration, chronic inflammation, or metabolic disorders like uncontrolled diabetes or dyslipidemia.
Management begins with accurate diagnosis: clinicians typically assess hematocrit, plasma viscosity, serum protein electrophoresis, and complete blood count, and may perform additional testing—including bone marrow evaluation or immunofixation—if a hematologic malignancy is suspected. Treatment is always directed at the root cause. For example, therapeutic phlebotomy is indicated in polycythemia vera to reduce red blood cell volume; plasmapheresis may be urgently required in symptomatic hyperviscosity syndrome associated with high-molecular-weight paraproteins; and aggressive rehydration is essential when hyperviscosity stems from volume depletion.
Lifestyle and supportive interventions also play a critical role. Patients are advised to maintain adequate hydration, avoid prolonged immobility, and manage comorbid conditions such as hypertension, hyperlipidemia, and insulin resistance. In select cases—particularly those with recurrent venous thromboembolism or persistently elevated viscosity despite optimal control of underlying disease—anticoagulation or antiplatelet therapy may be considered, though these are not routine and require individualized risk–benefit assessment.
Because hyperviscosity is rarely an isolated finding but rather a biomarker of systemic pathology, ongoing monitoring and multidisciplinary care—often involving hematology, cardiology, and primary care—are essential to mitigate long-term vascular complications and optimize clinical outcomes.
