This assessment begins with a detailed clinical history and physical examination, focusing on menstrual patterns, pelvic pain, bleeding abnormalities, prior gynecologic interventions, and comorbid conditions such as hypertension, diabetes, or cardiovascular disease.
Imaging plays a critical role: transvaginal ultrasound is routinely performed to assess uterine size, morphology, endometrial thickness, and detect structural abnormalities—including fibroids, adenomyosis, or ovarian masses. In cases of suspected deep infiltrating endometriosis, large or complex uterine pathology, or when surgical planning requires precise anatomical mapping, magnetic resonance imaging (MRI) may be indicated.
Endometrial sampling—via office-based endometrial biopsy or dilation and curettage (D&C)—is essential for women with abnormal uterine bleeding, especially those over age 45 or with risk factors for endometrial hyperplasia or carcinoma. Cervical cytology (Pap smear) and human papillomavirus (HPV) testing are reviewed to rule out cervical neoplasia; if abnormalities are present, colposcopy and biopsy are warranted before proceeding.
Standard preoperative labs include a complete blood count (CBC), basic metabolic panel, coagulation profile (PT/INR, aPTT), and type and screen—particularly in patients with known anemia, bleeding disorders, or anticipated significant blood loss. Additional testing, such as electrocardiography (ECG) or pulmonary function tests, may be ordered based on age, cardiac risk, or respiratory comorbidities.
Finally, shared decision-making is integral: surgeons discuss surgical approach (abdominal, laparoscopic, robotic-assisted, or vaginal), potential risks—including infection, hemorrhage, bladder or bowel injury, and long-term implications such as hormonal changes (if ovaries are removed) or impact on sexual function—and explore alternatives like medical management or less invasive procedures (e.g., endometrial ablation or myomectomy). A thorough informed consent process ensures alignment between patient values and clinical goals.
