AIDS, the final and most severe stage of HIV infection, occurs when the immune system has been critically damaged—typically defined by a CD4+ T-lymphocyte count falling below 200 cells/μL or the development of one or more AIDS-defining illnesses. During this phase, individuals are highly susceptible to opportunistic infections and malignancies that rarely affect immunocompetent people.
Common clinical manifestations include persistent fever, drenching night sweats, unexplained weight loss exceeding 10% of baseline body mass, chronic fatigue, and prolonged generalized lymphadenopathy. Gastrointestinal symptoms such as chronic diarrhea lasting more than one month may reflect opportunistic pathogens like Cryptosporidium, Mycobacterium avium complex (MAC), or cytomegalovirus (CMV).
Pulmonary involvement is frequent, with Pneumocystis jirovecii pneumonia (PCP) presenting as progressive dyspnea, nonproductive cough, and hypoxemia. Other respiratory pathogens—including Mycobacterium tuberculosis and Legionella species—may also cause severe disease. Neurological complications, such as HIV-associated dementia, toxoplasmosis encephalitis, or primary CNS lymphoma, can manifest as cognitive decline, motor deficits, seizures, or altered mental status.
Dermatologic findings often include recurrent herpes zoster (shingles), extensive molluscum contagiosum, or oral/esophageal candidiasis. Certain cancers—particularly Kaposi sarcoma (associated with human herpesvirus 8), invasive cervical cancer, and non-Hodgkin lymphoma—are classified as AIDS-defining conditions and signal profound immunosuppression.
Early diagnosis of HIV and prompt initiation of antiretroviral therapy (ART) remain the most effective strategies to prevent progression to AIDS. With consistent ART adherence, many individuals maintain durable viral suppression, preserve immune function, and avoid AIDS-related morbidity and mortality entirely.
