When tooth decay progresses deep into the dentin or pulp, bacteria can invade the pulp chamber, leading to pulpitis and, if left unmanaged, necrosis of the pulp tissue. This creates an ideal environment for bacterial proliferation, with inflammatory byproducts and pus accumulating at the root apex or within the periodontal ligament space.
The resulting periapical or periodontal abscess manifests clinically as a tender, fluctuant, and sometimes erythematous swelling on the gingiva—commonly described by patients as a “gum boil” or “gum blister.” Pain is usually moderate to severe, may be spontaneous or exacerbated by chewing or thermal stimuli, and can radiate to the jaw, ear, or temple. Systemic signs such as low-grade fever or malaise may accompany larger or more advanced infections.
Immediate clinical evaluation is essential. Diagnosis relies on a combination of patient history, clinical examination—including percussion and palpation tests—and radiographic imaging (e.g., periapical radiograph or cone-beam CT) to assess bone involvement and identify periapical radiolucency. Differential diagnoses include gingival abscesses (limited to soft tissue), periodontal abscesses (associated with deep periodontal pockets), and odontogenic cysts.
Definitive management requires source control: either root canal treatment to eliminate infected pulp tissue and seal the root canal system, or extraction if the tooth is non-restorable. Antibiotics—such as amoxicillin or clindamycin in penicillin-allergic patients—are adjunctive only and indicated when systemic symptoms, cellulitis, or immunocompromise is present; they do not replace definitive dental intervention. Delaying care risks complications including osteomyelitis, Ludwig’s angina, or septicemia.
Prevention hinges on early detection and treatment of caries, consistent oral hygiene, fluoride exposure, and routine dental examinations. Patients presenting with a painful gingival swelling over a decayed tooth should seek urgent dental evaluation to prevent progression and preserve both tooth structure and systemic health.
