Insomnia and cardiovascular health are more closely linked than many people realize. Emerging evidence from large-scale epidemiological studies and clinical trials indicates that chronic insomnia—characterized by difficulty falling asleep, staying asleep, or experiencing non-restorative sleep occurring at least three nights per week for three months or longer—is associated with a significantly increased risk of developing hypertension, coronary artery disease, heart failure, and stroke.
This association appears to be mediated through several interconnected physiological pathways. Persistent sleep disruption activates the sympathetic nervous system and dysregulates the hypothalamic-pituitary-adrenal axis, leading to elevated cortisol and catecholamine levels. These hormonal changes promote systemic inflammation, endothelial dysfunction, insulin resistance, and increased arterial stiffness—all established contributors to atherosclerosis and cardiac remodeling.
Notably, observational data suggest that individuals with insomnia have a 27% higher relative risk of incident cardiovascular disease compared with those without sleep disturbances, even after adjusting for traditional risk factors such as age, smoking, obesity, and physical inactivity. Furthermore, objective measures—including polysomnography and actigraphy—confirm that both short sleep duration (<6 hours per night) and poor sleep efficiency (<85%) independently predict adverse cardiac outcomes.
Clinicians should therefore consider insomnia not merely as a symptom but as a modifiable risk factor in cardiovascular risk assessment. Integrated management—including cognitive behavioral therapy for insomnia (CBT-I), judicious use of pharmacotherapy when indicated, and concurrent optimization of blood pressure, lipid profiles, and glycemic control—may offer synergistic benefits for long-term cardiac health.
