Cervical inflammation and cervical ectropion—often mistakenly referred to as “cervical erosion”—are common gynecological findings, but neither condition is infectious. Cervical inflammation refers to irritation or swelling of the cervix, which may result from infections (such as chlamydia, gonorrhea, or bacterial vaginosis), hormonal changes, chemical exposure (e.g., douches or spermicides), or mechanical irritation. Importantly, while some underlying causes—like sexually transmitted infections (STIs)—are transmissible, the inflammatory response itself is not contagious.
Cervical ectropion, a benign physiological variation frequently observed in adolescents, pregnant individuals, or those using hormonal contraception, occurs when columnar epithelium normally found inside the endocervical canal extends onto the ectocervix. This gives the cervix a red, granular appearance during speculum examination—but it is not disease, nor is it associated with infection or malignancy. It does not spread to others and requires no treatment unless accompanied by symptoms such as persistent discharge or postcoital bleeding, which would warrant evaluation for other etiologies.
Patients diagnosed with either finding should undergo appropriate diagnostic workup—including STI screening, Pap cytology, and HPV testing when indicated—to rule out treatable or potentially serious conditions. Management is guided by symptoms and underlying cause, not by the appearance alone. Reassurance and patient education are essential components of care, particularly given widespread misconceptions about these terms in both clinical and public health settings.
