Initial assessment begins with a thorough medical history, including dietary habits (e.g., high-purine food intake, alcohol consumption), medication use (e.g., diuretics, low-dose aspirin, immunosuppressants), family history of gout or kidney stones, and presence of metabolic risk factors such as hypertension, obesity, type 2 diabetes, or chronic kidney disease.
Diagnostic confirmation relies on a fasting serum uric acid measurement. Additional testing is often warranted to assess end-organ involvement and underlying drivers: renal function (serum creatinine, estimated glomerular filtration rate), complete blood count, comprehensive metabolic panel, lipid profile, and urinalysis. In patients with recurrent urolithiasis or suspected nephrolithiasis, non-contrast CT abdomen/pelvis or renal ultrasound may be indicated.
Referral to a specialist is recommended for patients with symptomatic gout, tophi, uric acid nephrolithiasis, progressive chronic kidney disease, or refractory hyperuricemia unresponsive to lifestyle modification and first-line pharmacotherapy (e.g., allopurinol or febuxostat). A rheumatologist manages inflammatory joint manifestations and long-term urate-lowering therapy; a nephrologist evaluates and treats uric acid–related renal injury or advanced kidney disease.
