Medically termed aural fullness, it can arise from several underlying causes, most frequently involving dysfunction of the Eustachian tube (also known as the pharyngotympanic or auditory tube), which connects the middle ear to the nasopharynx and normally equalizes pressure and drains secretions.
One of the most common triggers is Eustachian tube dysfunction (ETD), often linked to upper respiratory infections, allergic rhinitis, or sinusitis. Inflammation or mucosal swelling narrows the tube’s lumen, impairing ventilation and leading to negative middle ear pressure, fluid accumulation, and the characteristic闷 (stuffy) sensation. Barotrauma—such as during air travel or scuba diving—can produce similar symptoms due to rapid ambient pressure changes that the tube fails to compensate for.
Less common but important differential diagnoses include otosclerosis (abnormal bone remodeling around the stapes footplate), sudden sensorineural hearing loss (which may present with fullness preceding hearing decline), or even early-stage acoustic neuroma (vestibular schwannoma), particularly if accompanied by unilateral tinnitus, imbalance, or progressive hearing loss. Cerumen impaction—though typically associated with conductive hearing loss—can also mimic aural fullness, especially when impacted deep in the ear canal.
Initial management focuses on identifying and addressing the root cause. For ETD related to allergy or infection, intranasal corticosteroids and oral decongestants (used short-term) may reduce mucosal edema. Autoinflation techniques—like the Valsalva maneuver or Otovent® device—can help reopen the tube, though caution is advised in active upper respiratory infection to avoid iatrogenic inner ear injury. In persistent cases, tympanometry and audiometry are essential diagnostic tools; referral to an otolaryngologist is warranted if symptoms last beyond 3–4 weeks, worsen, or are accompanied by red flags such as vertigo, facial weakness, or asymmetric hearing loss.
Patients should avoid aggressive ear cleaning, prolonged use of nasal decongestant sprays (>3 days), or self-treatment with unproven remedies. Evidence-based care—not symptomatic masking—is key to preventing complications like chronic otitis media with effusion or permanent conductive hearing impairment.
