Vertigo and nausea are common, distressing symptoms that often occur together and can significantly impair daily function. These symptoms may arise from a range of underlying causes—some benign and self-limiting, others requiring prompt medical evaluation.
When vertigo (a false sensation of spinning or motion) is accompanied by nausea, the most frequent culprit is peripheral vestibular dysfunction—particularly benign paroxysmal positional vertigo (BPPV), vestibular neuritis, or labyrinthitis. BPPV results from displaced otoconia in the semicircular canals and typically triggers brief, intense vertigo with head movement. Vestibular neuritis involves acute, prolonged vertigo without hearing loss, usually due to viral inflammation of the vestibular nerve. Labyrinthitis shares similar features but also includes sensorineural hearing loss or tinnitus, reflecting concurrent cochlear involvement.
Less common—but clinically critical—causes include central nervous system disorders such as posterior circulation stroke, multiple sclerosis, or cerebellar lesions. Red-flag signs warranting urgent neuroimaging and specialist referral include sudden onset with headache, gait ataxia, dysarthria, nystagmus that changes direction with gaze, or neurological deficits like limb weakness or sensory loss.
Initial management focuses on symptom control and accurate diagnosis. Antihistamines (e.g., meclizine) or benzodiazepines (e.g., diazepam) may provide short-term relief for severe vertigo and nausea but should be used sparingly—typically no longer than 3–5 days—to avoid delaying vestibular compensation. For BPPV, canalith repositioning maneuvers (e.g., the Epley maneuver) are first-line and highly effective, with success rates exceeding 80% after one or two sessions.
Patients should be advised to avoid rapid head movements, maintain adequate hydration, and refrain from driving or operating heavy machinery until symptoms fully resolve. Persistent or recurrent vertigo warrants formal vestibular testing—including videonystagmography (VNG) or video head impulse testing (vHIT)—to differentiate peripheral from central etiologies and guide targeted rehabilitation or medical therapy.
