Herpes simplex virus (HSV) types 1 and 2 are responsible for the vast majority of cases—HSV-1 typically causes oral herpes (cold sores), while HSV-2 is most commonly associated with genital herpes. Both viruses establish lifelong latency in sensory nerve ganglia after primary infection and can reactivate periodically, leading to recurrent lesions. Other viruses—such as varicella-zoster virus (VZV), which causes chickenpox and shingles—produce clinically similar vesicular eruptions but are not classified as “herpes” in the strict sense; they belong to the same Herpesviridae family but cause distinct clinical syndromes. It is important to clarify that non-viral agents—including bacteria, fungi, or autoimmune processes—do not cause true herpes infections. Accurate diagnosis relies on clinical assessment and confirmatory testing such as PCR or viral culture, especially when atypical presentations occur.
