Visible hematuria—blood in the urine that is apparent to the naked eye—is always a clinically significant finding in women and warrants prompt medical evaluation. While occasional causes may be benign (e.g., vigorous exercise, certain medications like anticoagulants or cyclophosphamide, or transient urinary tract infections), sudden-onset gross hematuria should never be dismissed as trivial. It can signal serious underlying conditions, including urinary tract infections (especially if accompanied by dysuria, urgency, or suprapubic pain), kidney stones, glomerulonephritis, interstitial cystitis, or, importantly, urothelial malignancies such as bladder or renal pelvis cancer—even in the absence of pain or other symptoms. In women, gynecologic sources (e.g., endometriosis involving the bladder, cervical or vaginal lesions) must also be considered, though true urinary bleeding is typically distinguished from vaginal bleeding through careful history, physical examination, and urinalysis with microscopic evaluation. Diagnostic workup generally includes urinalysis with microscopy, urine culture, renal function testing, imaging (e.g., renal/bladder ultrasound or CT urogram), and often cystoscopy—particularly in patients over age 35 or those with risk factors such as smoking, occupational chemical exposure, or prior pelvic radiation. Early assessment is essential to identify treatable or time-sensitive pathology and to initiate appropriate management.
