Experiencing a sensation of heat or burning in the soles of the feet during nighttime rest—often described as “hot feet at night”—can be both uncomfortable and concerning. While occasional warmth may result from environmental factors such as heavy bedding or elevated room temperature, persistent or recurrent nocturnal foot warmth warrants clinical evaluation, as it may signal an underlying medical condition.
One of the most common causes is peripheral neuropathy—nerve damage frequently associated with diabetes mellitus. In diabetic neuropathy, impaired glucose metabolism leads to axonal degeneration and abnormal nerve signaling, often manifesting first in the distal extremities. Patients commonly report burning, tingling, or electric-like sensations in the feet, which may intensify when lying down due to reduced distraction and altered autonomic tone.
Other neurological contributors include vitamin B12 deficiency, chronic alcohol use disorder, uremic neuropathy (in advanced kidney disease), and hereditary neuropathies such as Charcot–Marie–Tooth disease. Autoimmune conditions like Sjögren’s syndrome or sarcoidosis can also cause small-fiber neuropathy presenting with isolated foot warmth or dysesthesia.
Vascular and inflammatory etiologies must also be considered. Erythromelalgia—a rare disorder characterized by episodic vasodilation, erythema, and burning pain—often worsens at night and is triggered by warmth or dependency. Similarly, complex regional pain syndrome (CRPS) may involve temperature dysregulation and allodynia in the affected limb.
Less commonly, nocturnal foot warmth may reflect systemic processes: hyperthyroidism (increasing basal metabolic rate and cutaneous blood flow), menopausal hormonal fluctuations, or certain medications—including selective serotonin reuptake inhibitors (SSRIs) and some chemotherapeutic agents known to induce neuropathic side effects.
A thorough diagnostic workup typically includes a detailed neurologic examination, serum testing for fasting glucose, HbA1c, vitamin B12, renal function, thyroid-stimulating hormone (TSH), and inflammatory markers. Electrophysiologic studies (nerve conduction studies and electromyography) or skin biopsy for intraepidermal nerve fiber density may be indicated based on clinical suspicion.
Management is tailored to the underlying cause: glycemic control in diabetes, B12 supplementation where deficient, immunomodulatory therapy for autoimmune neuropathies, or symptomatic relief with agents such as gabapentin, pregabalin, or low-dose tricyclic antidepressants. Nonpharmacologic strategies—including cooling foot baths before bed, breathable footwear, and maintaining a cool sleep environment—may provide adjunctive relief.
