Menstrual blood clots are a common and typically benign feature of many women’s periods. Small clots—often no larger than a quarter—are usually normal, especially during days of heaviest flow. They form when the uterine lining sheds rapidly and blood pools briefly in the uterus before being expelled, allowing time for coagulation factors to activate and fibrin networks to trap red blood cells.
However, an increase in clot size or frequency—particularly clots larger than a quarter (about 2.5 cm), occurring daily throughout the period, or accompanied by symptoms such as severe pelvic pain, prolonged bleeding (>7 days), or signs of anemia (fatigue, dizziness, pallor)—may signal an underlying condition. Common contributors include uterine fibroids, particularly submucosal or intramural types that distort the endometrial cavity; adenomyosis, where endometrial tissue invades the myometrium, leading to inefficient uterine contractions and impaired hemostasis; or hormonal imbalances such as unopposed estrogen exposure in polycystic ovary syndrome (PCOS) or perimenopause, resulting in excessive endometrial proliferation.
Less frequently, heavy menstrual bleeding with prominent clotting may reflect coagulopathies—including von Willebrand disease or platelet function disorders—or structural abnormalities like endometrial polyps or, rarely, endometrial hyperplasia or malignancy. A thorough evaluation should include a detailed menstrual history, pelvic examination, transvaginal ultrasound, and, when indicated, endometrial biopsy or hysteroscopy.
Management depends on etiology and patient goals. First-line medical therapy often includes combined hormonal contraceptives or progestin-only regimens to stabilize endometrial growth and reduce shedding volume. Tranexamic acid—a synthetic antifibrinolytic—can significantly decrease blood loss when taken during menses. For structural causes, surgical options such as hysteroscopic polypectomy, myomectomy, or endometrial ablation may be considered. Referral to a gynecologist is warranted if clots persist despite conservative measures or if red-flag symptoms emerge.
