Why Do Women Develop Hydrosalpinx?

Tubal hydrosalpinx refers to the accumulation of serous fluid within a fallopian tube, typically resulting from chronic inflammation—most commonly due to prior pelvic infection. The condition arises when inflammatory damage causes scarring and obstruction at the distal (fimbrial) end of the tube, trapping secretions produced by the tubal epithelium. As fluid builds up, the tube becomes dilated and sausage-shaped, often losing its normal architecture and function.

Chlamydia trachomatis and Neisseria gonorrhoeae are the leading infectious agents implicated in the development of hydrosalpinx. However, other pathogens—including Mycoplasma genitalium and aerobic/anaerobic bacteria associated with pelvic inflammatory disease (PID)—can also contribute. In some cases, hydrosalpinx follows surgical procedures such as cesarean delivery or appendectomy, or occurs after endometriosis-related adhesions or tubal ligation reversal.

Notably, many patients remain asymptomatic for years; diagnosis is frequently incidental during fertility evaluation or imaging for unrelated concerns. When symptoms do occur, they may include chronic lower abdominal pain, abnormal vaginal discharge, or infertility—particularly because hydrosalpinx impairs oocyte pickup, disrupts embryo transport, and introduces a toxic fluid environment that can negatively affect endometrial receptivity and implantation success.

Imaging modalities such as transvaginal ultrasound and hysterosalpingography (HSG) are first-line diagnostic tools, though laparoscopy remains the gold standard for definitive confirmation and assessment of associated pelvic pathology.
