Amniotic fluid begins to form shortly after conception, typically around the end of the first week following fertilization. As the blastocyst implants into the uterine lining, extraembryonic membranes—including the amnion—start developing. By approximately day 12 post-fertilization, a small amount of fluid accumulates within the amniotic cavity, marking the earliest stage of amniotic fluid formation.
The volume of amniotic fluid increases progressively throughout pregnancy. During the second trimester, production accelerates significantly, primarily driven by fetal urine output and pulmonary secretions. By mid-gestation (around 18–20 weeks), ultrasound can reliably detect and quantify amniotic fluid volume using measurements such as the amniotic fluid index (AFI) or single deepest pocket (SDP).
Clinically, the presence of adequate amniotic fluid is essential for fetal development—it supports lung maturation, permits musculoskeletal growth, cushions against mechanical stress, and helps maintain thermal and biochemical homeostasis. Abnormalities in volume—either oligohydramnios (low fluid) or polyhydramnios (excess fluid)—may signal underlying fetal, placental, or maternal conditions and warrant further evaluation.
While amniotic fluid is present from early embryogenesis, it becomes both quantitatively and functionally significant beginning in the late first trimester and continues to evolve in composition and volume until term.
