While occasional sleep disruption and transient cardiovascular symptoms may stem from stress or caffeine intake, persistent co-occurrence warrants careful evaluation to distinguish between primary sleep disorders, anxiety-related autonomic arousal, and underlying cardiac pathology.
First-line assessment should include a detailed history focusing on symptom timing (e.g., whether chest discomfort occurs exclusively at night, upon awakening, or during nocturnal awakenings), associated features (shortness of breath, diaphoresis, dizziness), and risk factors for cardiovascular disease—including hypertension, diabetes, dyslipidemia, smoking history, and family history of premature coronary artery disease. Objective monitoring—such as ambulatory electrocardiography (Holter monitoring) or an event recorder—may be indicated if arrhythmias like nocturnal atrial fibrillation or ventricular ectopy are suspected.
Importantly, insomnia itself can trigger sympathetic nervous system activation, leading to elevated heart rate, blood pressure variability, and increased myocardial oxygen demand—potentially unmasking or exacerbating subclinical coronary ischemia. Conversely, conditions such as obstructive sleep apnea (OSA) frequently present with both fragmented sleep and nocturnal cardiac strain due to recurrent hypoxemia, intrathoracic pressure swings, and intermittent sympathetic surges. Polysomnography remains the gold standard for diagnosing OSA in patients with this symptom complex.
Management must be individualized. Cognitive behavioral therapy for insomnia (CBT-I) is the first-line nonpharmacologic intervention and has demonstrated efficacy not only for improving sleep continuity but also for reducing autonomic hyperarousal and associated cardiovascular stress markers. Pharmacologic approaches—such as short-term use of sedative-hypnotics or melatonin receptor agonists—should be weighed against potential risks, particularly in older adults or those with comorbid cardiopulmonary disease. When cardiac symptoms persist despite optimized sleep management, referral to cardiology for further risk stratification—including stress testing or coronary imaging—may be warranted.
Patients experiencing new-onset or worsening nocturnal chest discomfort with insomnia should seek prompt medical evaluation. Red flags include exertional or recumbent angina, syncope, orthopnea, or documented arrhythmias—any of which necessitate urgent triage to rule out acute coronary syndrome or other life-threatening conditions.
