While occasionally benign and transient, persistent or recurrent upper abdominal bloating warrants careful clinical evaluation, as it may signal underlying gastrointestinal, gynecologic, hepatic, pancreatic, or even cardiac conditions.
Physiologically, upper abdominal bloating can result from delayed gastric emptying (gastroparesis), small intestinal bacterial overgrowth (SIBO), functional dyspepsia, or aerophagia—particularly in individuals who eat rapidly, chew gum frequently, or experience anxiety-related hyperventilation. Hormonal fluctuations across the menstrual cycle, especially elevated progesterone levels during the luteal phase, may contribute to smooth muscle relaxation and slowed motilin-driven gut transit, predisposing some women to postprandial fullness and bloating.
Clinically significant causes require differentiation. Peptic ulcer disease, chronic gastritis, or Helicobacter pylori infection may present with epigastric discomfort and early satiety. Pancreatic exocrine insufficiency—especially in patients with prior pancreatitis or cystic fibrosis—can lead to maldigestion and gas accumulation. Hepatobiliary disorders, including nonalcoholic fatty liver disease (NAFLD) or cholestatic syndromes, may manifest with right upper quadrant heaviness and bloating. Importantly, ovarian malignancies—including epithelial ovarian cancer—can cause subtle but progressive upper abdominal distension due to ascites or omental involvement, often accompanied by nonspecific symptoms such as early satiety, urinary frequency, or unexplained weight loss.
Initial assessment should include a detailed history focusing on symptom chronology, dietary triggers (e.g., high-FODMAP foods, carbonated beverages), medication use (e.g., opioids, anticholinergics), menstrual and reproductive history, and red-flag features—including unintentional weight loss, nocturnal pain, dysphagia, hematemesis, melena, or palpable abdominal mass. Physical examination must assess for hepatosplenomegaly, fluid wave, shifting dullness, and pelvic tenderness or adnexal enlargement.
First-line investigations typically include complete blood count, comprehensive metabolic panel, fasting glucose, HbA1c, liver enzymes, amylase/lipase, and serologic testing for H. pylori. Transabdominal ultrasound is appropriate for initial structural evaluation of the liver, gallbladder, pancreas, and pelvis. In premenopausal women with suspected functional dyspepsia and negative initial workup, a trial of proton pump inhibitor therapy or prokinetic agents may be considered. Postmenopausal women—or those with persistent symptoms despite empiric management—require further imaging, such as contrast-enhanced CT or MRI, and referral to gastroenterology or gynecologic oncology as clinically indicated.
Nonpharmacologic strategies remain foundational: mindful eating, avoidance of carbonated drinks and chewing gum, reduction of fermentable carbohydrates (e.g., fructose, lactose, polyols), and stress mitigation through cognitive behavioral therapy or diaphragmatic breathing techniques. Patients should be counseled that while occasional bloating is common, new-onset, progressive, or symptom-complex upper abdominal distension merits timely medical evaluation to exclude serious pathology.
