Endocrine disorders represent a frequent contributor. Hypothyroidism, characterized by insufficient thyroid hormone production, commonly manifests as lethargy, weight gain, cold intolerance, and slowed cognition. Similarly, testosterone deficiency—particularly in aging men or those with pituitary or testicular dysfunction—can lead to diminished energy, reduced motivation, decreased muscle mass, and sleep disturbances. Adrenal insufficiency, though rarer, may present with profound fatigue, orthostatic hypotension, and electrolyte imbalances.
Sleep-related conditions are another major category. Obstructive sleep apnea (OSA) is highly prevalent among men, especially those who are overweight or have anatomical upper airway narrowing. Repeated nocturnal hypoxia and sleep fragmentation result in nonrestorative sleep and daytime somnolence. Insomnia, circadian rhythm disorders, and restless legs syndrome also significantly impair sleep quality and contribute to persistent fatigue.
Psychiatric conditions—including major depressive disorder, generalized anxiety disorder, and chronic stress—frequently express physically through fatigue, even in the absence of overt mood symptoms. Fatigue may be the predominant or presenting complaint, particularly in men who underreport emotional distress.
Medical comorbidities must also be considered. Anemia—whether due to iron deficiency, chronic disease, or vitamin B12/folate insufficiency—reduces oxygen-carrying capacity and induces exertional fatigue. Cardiovascular disease, chronic kidney disease, liver dysfunction, and uncontrolled diabetes mellitus all disrupt metabolic homeostasis and energy metabolism. Autoimmune conditions such as rheumatoid arthritis or systemic lupus erythematosus often involve cytokine-mediated fatigue that is disproportionate to objective disease activity.
Lifestyle factors play a substantial role. Sedentary behavior, poor dietary patterns (e.g., high glycemic load, inadequate protein or micronutrients), excessive alcohol intake, and nicotine use impair mitochondrial function and cellular energy production. Chronic sleep deprivation—even modest reductions over time—disrupts hypothalamic-pituitary-adrenal axis regulation and increases inflammatory biomarkers.
Medication side effects are an underrecognized cause. Beta-blockers, antihistamines, certain antidepressants (e.g., tricyclics), opioids, and statins have well-documented associations with fatigue. Polypharmacy further amplifies this risk.
A thorough diagnostic approach includes detailed history-taking (focusing on onset, diurnal pattern, associated symptoms, sleep habits, psychosocial stressors, and medication review), physical examination, and targeted investigations—such as complete blood count, comprehensive metabolic panel, thyroid-stimulating hormone, ferritin, vitamin D, testosterone (morning total and free), HbA1c, and sleep study when indicated. Management is etiology-specific and may involve hormonal replacement, cognitive behavioral therapy for insomnia or depression, continuous positive airway pressure for OSA, nutritional optimization, or pharmacologic adjustment.
