Unlike persistent or high-grade fevers, intermittent low-grade elevations in body temperature often reflect subtle, self-limited, or subclinical processes rather than acute systemic infection or malignancy.
Among the most frequent causes are mild viral upper respiratory infections, such as those caused by rhinoviruses or endemic coronaviruses, which may produce only transient, low-intensity febrile responses—particularly in immunocompetent adults. Similarly, localized bacterial infections—including chronic sinusitis, dental abscesses, or early-stage urinary tract infections—can manifest with isolated low-grade fever without prominent constitutional symptoms.
Non-infectious inflammatory conditions also merit consideration. Subclinical autoimmune activity, as seen in early-stage rheumatoid arthritis or systemic lupus erythematosus, may present with episodic low-grade fever before overt joint or serologic findings emerge. Granulomatous diseases—including sarcoidosis or latent tuberculosis—can likewise cause intermittent temperature elevation due to low-level granulomatous inflammation.
Other potential contributors include medication effects (e.g., drug-induced hypersensitivity or serotonin syndrome), endocrine disturbances such as subacute thyroiditis, and physiological variants like “temperature dysregulation” in perimenopausal individuals or those with autonomic dysfunction. In rare cases, occult neoplasms—particularly lymphomas or renal cell carcinomas—may present with paraneoplastic low-grade fever, though this is typically accompanied by other systemic “B symptoms” (e.g., night sweats, unintentional weight loss, fatigue).
Clinical evaluation should begin with a detailed history—including timing, duration, diurnal pattern, associated symptoms, medication use, travel, and psychosocial stressors—followed by targeted physical examination. Routine initial testing often includes complete blood count, comprehensive metabolic panel, C-reactive protein or erythrocyte sedimentation rate, urinalysis, and chest radiography. Further investigation is guided by clinical suspicion and should avoid overtesting in otherwise asymptomatic, low-risk individuals.
