While occasional variation in menstrual bleeding is normal, persistently light periods (hypomenorrhea) may signal underlying physiological or pathological changes. Common contributors include hormonal imbalances—particularly those involving estrogen, progesterone, thyroid hormones, or prolactin—as well as structural factors such as intrauterine adhesions (Asherman syndrome), endometrial atrophy, or prior uterine instrumentation like dilation and curettage (D&C). Polycystic ovary syndrome (PCOS), premature ovarian insufficiency, and hypothalamic amenorrhea related to stress, excessive exercise, or low body weight are also frequent endocrine causes. Additionally, certain medications—including hormonal contraceptives (especially progestin-only methods), antipsychotics, and some antidepressants—can suppress endometrial proliferation and reduce menstrual shedding. Age-related transitions, such as perimenopause, often manifest with declining estrogen levels and increasingly sparse menses. Evaluation typically begins with a detailed history and physical examination, followed by targeted investigations: serum hormone panels (FSH, LH, estradiol, TSH, prolactin), pelvic ultrasound, and, when indicated, hysteroscopy to assess endometrial cavity integrity.
It is important to distinguish clinically significant hypomenorrhea from normal fluctuations—for example, lighter bleeding during the first few cycles after initiating hormonal contraception or in the early years following menarche. However, new-onset or progressive reduction in flow—especially when accompanied by infertility, cycle irregularity, hot flashes, vaginal dryness, or galactorrhea—warrants timely gynecologic assessment. Management is tailored to the underlying etiology: hormone replacement therapy for hypoestrogenic states, surgical lysis of intrauterine adhesions, lifestyle modification for functional hypothalamic suppression, or adjustment of medication regimens where appropriate. Early diagnosis not only addresses symptomatic concerns but also helps identify conditions with broader reproductive or systemic implications.
