What Causes Facial Redness and Yellowing?

Facial redness and yellowing can signal a range of underlying conditions—from benign, transient reactions to serious systemic disease—and warrant careful clinical evaluation. Persistent erythema (redness) may reflect inflammatory dermatoses such as rosacea, contact dermatitis, or seborrheic dermatitis; vascular phenomena like telangiectasia or flushing disorders; or systemic processes including lupus erythematosus or carcinoid syndrome. Concurrent jaundice—manifested as yellow discoloration of the skin and sclera—is a hallmark sign of hyperbilirubinemia, most commonly due to hepatobiliary dysfunction (e.g., hepatitis, cirrhosis, cholestasis) or hemolysis. When facial redness co-occurs with jaundice, clinicians must assess for overlapping etiologies: chronic liver disease may cause both portal hypertension–related facial flushing and impaired bilirubin conjugation, while certain medications (e.g., anabolic steroids, chlorpromazine) or metabolic disorders (e.g., Gilbert syndrome, Dubin-Johnson syndrome) can produce mixed pigmentary changes. A thorough history—including onset, duration, associated symptoms (pruritus, fatigue, dark urine, pale stools), medication use, alcohol intake, and family history—alongside physical examination and targeted laboratory testing (liver function tests, complete blood count, viral serologies, autoimmune markers) is essential to differentiate between cutaneous, hepatic, hematologic, and endocrine causes.
