What Causes Black Streaks in Vaginal Discharge After Intercourse?

Black or dark brown streaks in vaginal discharge following sexual intercourse can be concerning, but they often reflect benign, transient causes rather than serious pathology. This discoloration typically represents old blood—hemoglobin that has undergone oxidation and degradation—rather than fresh bleeding. Common explanations include minor cervical trauma during intercourse, such as microabrasions or friable cervical ectropion (also known as cervical erosion), particularly in individuals using hormonal contraception or those with elevated estrogen levels.

Other potential contributors include endometrial shedding related to perimenstrual timing, retained menstrual blood expelled after coitus, or cervical polyps—benign, vascular growths that may bleed easily upon contact. Less commonly, persistent or recurrent postcoital spotting with dark-tinged discharge warrants evaluation for cervical dysplasia, chronic cervicitis, or, rarely, early-stage cervical cancer—especially in individuals with risk factors such as HPV infection, smoking, or inadequate cervical cancer screening history.

While isolated, self-limited episodes are usually not urgent, clinical assessment is recommended if the finding recurs, is accompanied by other symptoms (e.g., pelvic pain, foul odor, abnormal bleeding between periods, or postmenopausal bleeding), or occurs in individuals overdue for cervical cytology or HPV testing. A comprehensive evaluation typically includes speculum examination, bimanual exam, Pap smear, and—if indicated—HPV co-testing or colposcopy with directed biopsy.
