Initial management follows the RICE protocol: Rest to prevent further tissue damage; Ice applied for 15–20 minutes every 2–3 hours during the first 48–72 hours to reduce inflammation and pain; Compression with an elastic bandage to minimize swelling; and Elevation of the affected arm above heart level when possible, though practicality is limited given anatomical constraints.
After the acute phase, gradual rehabilitation becomes essential. Physical therapy focuses on restoring range of motion, strengthening the dynamic stabilizers of the shoulder joint—including the supraspinatus, infraspinatus, teres minor, and subscapularis—and retraining neuromuscular control. Manual therapy, scapular stabilization exercises, and progressive resistance training are evidence-based components of effective recovery protocols.
Nonsteroidal anti-inflammatory drugs (NSAIDs), such as ibuprofen or naproxen, may be used short-term for symptomatic relief, but prolonged use is discouraged due to potential soft-tissue healing impairment. Corticosteroid injections are generally avoided in isolated muscle strains, as they carry risks of tendon weakening and delayed regeneration without proven benefit for pure myofascial injury.
Return-to-activity timelines vary based on strain severity: Grade I (mild microtears) often resolves within 1–2 weeks; Grade II (partial tearing with moderate functional loss) may require 3–6 weeks; and Grade III (complete rupture) is rare in isolated muscle strains but necessitates specialist evaluation and possibly surgical consultation. Persistent pain beyond 6 weeks, night pain, or progressive weakness warrants imaging—such as diagnostic ultrasound or MRI—to rule out concomitant tendinopathy, partial-thickness tears, or labral involvement.
