Discoid eczema—also known as nummular eczema or discoid dermatitis—is a chronic, inflammatory skin condition characterized by well-demarcated, coin-shaped (nummular) plaques. While it most commonly affects adults, particularly those aged 50–70 years, infants and young children can also develop this variant, albeit less frequently. In pediatric cases, the presentation may overlap with atopic dermatitis, but discoid eczema is distinguished by its sharply circumscribed, round or oval lesions—often with central crusting, scaling, and peripheral erythema.
When thick, adherent crusts form on affected areas in infants, this typically reflects an acute or subacute flare, often complicated by secondary bacterial colonization—most commonly with *Staphylococcus aureus*. Crusting results from serous exudate drying on the skin surface and may be accompanied by pruritus, oozing, or localized swelling. Importantly, thick crusting should not be forcibly removed, as this risks epidermal disruption, microtrauma, and increased infection risk.
Management begins with gentle, non-irritating emolliation using fragrance-free, hypoallergenic ointments or creams applied liberally and frequently—especially after brief, lukewarm baths. For established crusts, soaking with lukewarm water and mild, soap-free cleansers for 5–10 minutes softens the scale, allowing gradual, passive removal during subsequent moisturizing. Topical corticosteroids—low-potency agents such as hydrocortisone 1% ointment—are first-line for active inflammation, applied once daily for limited duration (typically 7–14 days), under pediatric dermatologic guidance. In cases with clinical signs of infection—such as increased erythema, warmth, purulent discharge, or systemic symptoms—targeted topical or oral antibiotics may be warranted.
Long-term prevention hinges on consistent skin barrier support: avoiding harsh soaps, wool or synthetic fabrics, overheating, and environmental irritants. Parents should be counseled to monitor for recurrence and seek prompt evaluation if lesions spread, fail to improve with standard therapy, or show signs of worsening inflammation or infection. Referral to a pediatric dermatologist is recommended for atypical presentations, treatment resistance, or diagnostic uncertainty—particularly to differentiate discoid eczema from tinea corporis, psoriasis, or contact dermatitis.
