Symptoms often include unintended weight loss, chronic diarrhea, steatorrhea (fatty, foul-smelling stools), bloating, fatigue, and deficiencies in fat-soluble vitamins (A, D, E, K) or micronutrients like iron, folate, and vitamin B12.
Effective management begins with accurate diagnosis—typically involving stool studies (e.g., fecal elastase-1, quantitative fecal fat), serologic testing (e.g., tissue transglutaminase antibodies for celiac disease), imaging (CT or MR enterography), and endoscopic evaluation with biopsy when indicated. Breath tests may be used to assess carbohydrate malabsorption or SIBO.
Treatment is etiology-specific and multifaceted. For celiac disease, strict lifelong adherence to a gluten-free diet remains the cornerstone. In pancreatic insufficiency, oral pancreatic enzyme replacement therapy (PERT) taken with meals and snacks is essential to restore digestion of fats, proteins, and carbohydrates. Antibiotics such as rifaximin are employed for confirmed SIBO, while immunomodulators or biologics may be required for inflammatory bowel disease–related malabsorption.
Nutritional rehabilitation is integral: individualized supplementation addresses specific deficiencies—such as intramuscular vitamin B12 for pernicious anemia–like absorption failure, high-dose oral vitamin D3 for deficiency, or parenteral iron for severe iron-refractory cases. Dietitians play a critical role in optimizing caloric intake, recommending medium-chain triglyceride (MCT) oils for easier fat absorption, and avoiding trigger foods that exacerbate symptoms.
Long-term monitoring—including periodic assessment of nutritional biomarkers, bone mineral density, and symptom burden—is vital to prevent complications such as osteoporosis, neuropathy, anemia, and growth failure in pediatric patients. Early recognition and coordinated care across gastroenterology, nutrition, and primary care significantly improve quality of life and clinical outcomes.
