Scabies is a highly contagious skin infestation caused by the microscopic mite Sarcoptes scabiei var. hominis. Transmission occurs almost exclusively through prolonged, direct skin-to-skin contact—typically lasting 15 to 20 minutes—with an infected individual. This mode of transmission explains why scabies spreads readily within households, among sexual partners, and in crowded or institutional settings such as nursing homes, shelters, and correctional facilities.
While less common, indirect transmission can occur via fomites—especially in cases of crusted (norwegian) scabies, where thousands to millions of mites may be present on the skin. Contaminated bedding, clothing, or towels may harbor viable mites for up to 48–72 hours under cool, humid conditions. However, casual contact—such as handshakes, brief hugs, or sharing utensils—is not considered a significant risk for acquiring typical (non-crusted) scabies.
It is important to note that scabies mites cannot jump, fly, or burrow through intact clothing; they require sustained epidermal contact to establish infestation. Additionally, humans are the only natural host for S. scabiei var. hominis, and animal scabies mites (e.g., from dogs or cats) cause only transient, self-limiting pruritus in humans and do not lead to persistent infestation.
Early recognition of transmission patterns is critical for effective outbreak control. Public health interventions—including simultaneous treatment of all close contacts, thorough laundering of personal items at high temperatures (>50°C), and environmental decontamination—are essential to interrupt transmission chains and prevent reinfestation.
