Recovery timelines vary depending on several clinical factors, including patient age, fracture location and displacement, associated soft-tissue injury, and overall health status. In general, non-surgical management—which accounts for approximately 80–90% of clavicle fractures—is the standard of care for minimally displaced midshaft fractures. With conservative treatment (typically involving sling immobilization, pain control, and progressive physical therapy), most patients experience significant pain reduction within 2–4 weeks. Clinical union—defined by absence of tenderness at the fracture site and radiographic evidence of bridging callus—usually occurs by 6–8 weeks in adults and somewhat earlier in children.
Full functional recovery, however, extends beyond bony healing. Return to unrestricted activities—including contact sports or heavy lifting—typically requires 12–16 weeks, contingent upon restoration of full shoulder range of motion, scapular stability, and rotator cuff strength. Delayed union (failure to progress toward healing by 3 months) or nonunion (lack of healing after 6 months) occurs in roughly 5–10% of cases, more commonly with highly displaced or comminuted fractures, and may warrant surgical evaluation.
Patient education is critical: early gentle mobilization under guidance helps prevent adhesive capsulitis and muscle atrophy, while premature return to strenuous activity increases re-injury risk.
