Muscle and fascial pain—often referred to clinically as myofascial pain syndrome (MPS)—is a common, yet frequently underrecognized, musculoskeletal condition characterized by localized or referred pain originating from hyperirritable spots in skeletal muscle known as myofascial trigger points. These trigger points may develop due to acute trauma, repetitive microtrauma, poor posture, prolonged immobility, or psychological stress.
Diagnosis relies primarily on physical examination: clinicians identify taut bands within affected muscles and elicit localized tenderness, referred pain patterns, and a local twitch response upon manual palpation. Imaging and laboratory tests are typically normal and not diagnostic; they serve mainly to exclude other conditions such as radicular pain, inflammatory arthritis, or metabolic myopathies.
First-line management emphasizes nonpharmacologic interventions. Evidence-based approaches include targeted stretching, progressive strengthening exercises, and manual therapies such as ischemic compression, dry needling, or myofascial release techniques performed by trained physical therapists or certified practitioners. Patient education on ergonomics, sleep hygiene, and stress modulation is integral to long-term symptom control.
Pharmacologic options play a supportive role. Short-term use of nonsteroidal anti-inflammatory drugs (NSAIDs) may alleviate associated inflammation, though their efficacy for pure myofascial pain is modest. Muscle relaxants are generally discouraged due to limited evidence and risk of sedation or dependency. In select cases—particularly when conservative measures fail—trigger point injections with saline, local anesthetics, or corticosteroids may be considered, though corticosteroids are used sparingly given concerns about tissue atrophy.
Prognosis is generally favorable with multidisciplinary care. Most patients experience meaningful improvement within weeks to months. However, chronicity can occur if contributing factors—such as persistent biomechanical strain or unaddressed psychosocial comorbidities like anxiety or depression—are not systematically addressed. A holistic, patient-centered approach remains the cornerstone of effective management.
