How is ganglion cyst treated with medication?

Tendon sheath cysts, more accurately referred to as ganglion cysts, are benign, fluid-filled sacs that commonly arise from joints or tendon sheaths—most frequently at the wrist, hand, ankle, or foot. Importantly, there is no pharmacologic (medication-based) treatment proven effective for ganglion cysts. These cysts contain viscous, gelatinous synovial fluid and lack a true epithelial lining; they are not inflammatory or infectious in nature, so anti-inflammatory drugs (e.g., NSAIDs), antibiotics, or corticosteroids administered systemically or orally do not alter their natural course or promote resolution.

Current evidence-based management focuses on observation, aspiration, or surgical excision—not medication. Many ganglion cysts are asymptomatic and resolve spontaneously over time; therefore, watchful waiting with reassurance is appropriate for non-bothersome lesions. For symptomatic cysts causing pain, limitation of motion, or cosmetic concern, ultrasound-guided needle aspiration—often combined with corticosteroid injection into the cyst cavity—may be attempted. However, recurrence rates after aspiration alone exceed 50%, and adding corticosteroid does not consistently improve outcomes. Surgical excision offers the lowest recurrence rate (approximately 5–10%) but carries risks including infection, nerve injury, and scar formation.

Patients should be counseled that topical agents, oral supplements, herbal remedies, or “anti-cyst” medications marketed online have no scientific support and may delay appropriate evaluation or lead to unnecessary expense or harm. Referral to a hand surgeon or orthopedic specialist is recommended when diagnosis is uncertain, symptoms are progressive, or conservative measures fail.
