One common yet underrecognized contributor is dietary habit—particularly frequent consumption of fermentable carbohydrates, including seemingly innocuous items like fruit juices, flavored yogurts, dried fruits, and even sugar-free sodas containing acidic additives. When ingested multiple times daily, these foods lower intraoral pH, creating an extended demineralization window during which enamel loses calcium and phosphate ions. Saliva normally buffers this acidity and promotes remineralization, but its protective capacity can be overwhelmed by high-frequency exposure.
Reduced salivary flow—xerostomia—is another critical factor. It may result from medications (e.g., anticholinergics, antidepressants, antihypertensives), autoimmune conditions such as Sjögren’s syndrome, radiation therapy to the head and neck, or chronic mouth breathing. Saliva not only neutralizes acid but also delivers antimicrobial peptides and calcium phosphate complexes essential for enamel repair; its deficiency significantly elevates caries risk.
Anatomical considerations also play a role. Deep occlusal fissures, enamel hypoplasia, or malpositioned teeth can create plaque-retentive niches that resist mechanical cleaning—even with diligent home care. In such cases, preventive interventions like sealants or minimally invasive restorations may be indicated.
Additionally, microbial dysbiosis—specifically elevated levels of Streptococcus mutans and Lactobacillus species—can predispose individuals to aggressive caries progression. This imbalance may stem from early childhood transmission (e.g., via shared utensils or tasting food), prolonged antibiotic use, or disruptions in oral microbiome diversity.
Clinicians stress that identifying the root cause requires a comprehensive caries risk assessment—not just clinical examination, but also evaluation of diet logs, medication history, salivary flow testing, and sometimes microbiological analysis. Management must therefore be personalized: optimizing fluoride exposure (e.g., prescription-strength toothpaste), recommending pH-neutralizing rinses, addressing dry mouth medically or behaviorally, and implementing targeted preventive strategies aligned with individual biologic and lifestyle profiles.
