While nasal discharge can occur in some individuals with SARS-CoV-2 infection, it is not considered a hallmark or frequent symptom. According to data from the World Health Organization (WHO) and large-scale cohort studies, the most prevalent symptoms of acute COVID-19 include fever, dry cough, fatigue, and loss of taste or smell (anosmia or dysgeusia). Rhinorrhea appears in approximately 5–10% of confirmed cases, particularly among children and mild adult infections, but it remains significantly less common than in viral upper respiratory infections such as rhinovirus or seasonal influenza.
Clinically, when rhinorrhea does occur in COVID-19, it is often accompanied by other systemic features—such as myalgia, headache, or gastrointestinal symptoms—and tends to be less prominent than nasal congestion or postnasal drip. Importantly, isolated rhinorrhea—without fever, cough, or systemic signs—is more suggestive of allergic rhinitis or a common cold rather than acute SARS-CoV-2 infection. Diagnostic evaluation should therefore consider the full constellation of symptoms, exposure history, and, when indicated, molecular testing (e.g., RT-PCR) or rapid antigen assays.
Healthcare providers are advised to maintain a high index of suspicion for COVID-19 even in atypical presentations, especially amid co-circulating respiratory pathogens. However, the presence of rhinorrhea alone should not drive testing or isolation decisions without corroborating clinical context. Public health messaging continues to emphasize that symptom-based screening must account for variability across age groups, vaccination status, and viral variants—underscoring the importance of objective diagnostics over symptom reliance alone.
