Hepatic cirrhosis with gastric varices—particularly those located in the fundus of the stomach—is a serious, potentially life-threatening complication of advanced liver disease. While the underlying cirrhosis itself is generally irreversible, gastric varices can be effectively managed to reduce bleeding risk and improve survival. Treatment focuses not on “curing” the varices per se, but on preventing rupture and controlling acute hemorrhage when it occurs.
First-line management includes non-selective beta-blockers (e.g., propranolol or carvedilol), which lower portal pressure and significantly reduce the risk of first-time variceal bleeding. For patients who cannot tolerate beta-blockers—or those with high-risk features such as large varices or red wale signs—endoscopic therapy is indicated. Unlike esophageal varices, gastric fundal varices are less amenable to band ligation due to anatomical constraints; instead, endoscopic injection of cyanoacrylate glue (e.g., N-butyl-2-cyanoacrylate) is the preferred intervention and has demonstrated superior efficacy in preventing rebleeding.
In cases of refractory or recurrent bleeding, transjugular intrahepatic portosystemic shunt (TIPS) may be considered. TIPS reduces portal pressure by creating a shunt between the hepatic vein and portal vein, offering durable control in selected patients—though it carries risks including hepatic encephalopathy and heart failure, particularly in those with compromised cardiac or liver reserve.
Ultimately, long-term outcomes depend heavily on addressing the root cause: ongoing alcohol use, viral hepatitis (especially HBV or HCV), or metabolic dysfunction–associated steatotic liver disease (MASLD). Liver transplantation remains the only definitive treatment for decompensated cirrhosis and offers the best chance for sustained resolution of portal hypertension and its complications—including gastric varices.
With timely diagnosis, multidisciplinary care, and adherence to evidence-based interventions, many patients achieve stable disease control and avoid life-threatening hemorrhage. However, gastric varices signal advanced portal hypertension and warrant close surveillance, regular endoscopic monitoring, and coordinated management by hepatologists, gastroenterologists, and interventional radiologists.
