At this threshold, the brain—whose primary fuel source is glucose—experiences acute energy failure. Neuronal dysfunction manifests rapidly as confusion, agitation, seizures, and loss of consciousness. Prolonged exposure to such profound hypoglycemia can trigger excitotoxic neuronal damage, cerebral edema, and ultimately, brainstem herniation.
Mortality risk increases substantially when hypoglycemia persists for more than 15–30 minutes without correction. Factors that elevate fatality risk include advanced age, preexisting cardiovascular disease, concurrent use of beta-blockers (which mask adrenergic warning signs), and impaired counterregulatory hormone responses—common in long-standing type 1 diabetes or autonomic neuropathy.
Clinical management requires immediate intravenous dextrose (typically 25 g of 50% dextrose) or intramuscular glucagon if IV access is unavailable. Following stabilization, patients require continuous glucose monitoring, evaluation for underlying causes (e.g., insulin overdose, sulfonylurea toxicity, insulinoma, adrenal insufficiency), and individualized glycemic targets to prevent recurrence.
Prevention hinges on patient education, regular self-monitoring of blood glucose, recognition of early symptoms (e.g., tremor, diaphoresis, palpitations), and timely carbohydrate intake. For high-risk individuals, continuous glucose monitoring systems with predictive low-glucose alerts and automated insulin suspension features have demonstrated efficacy in reducing severe hypoglycemic events.
