Early-stage HIV infection—often referred to as acute or primary HIV infection—typically occurs within two to four weeks after exposure to the virus. During this phase, some individuals develop a flu-like illness known as acute retroviral syndrome, characterized by fever, sore throat, fatigue, myalgia, and generalized lymphadenopathy. While skin manifestations are relatively common in this stage, discrete brown macules or patches are not a hallmark feature of acute HIV infection.
Brown pigmented lesions appearing early in the course of HIV disease are more frequently associated with other conditions—including drug reactions (e.g., antiretroviral therapy–induced hyperpigmentation), post-inflammatory changes, or unrelated dermatoses such as lentigines or melasma. In advanced, untreated HIV infection—particularly when CD4+ T-lymphocyte counts fall below 200 cells/μL—certain opportunistic infections and malignancies may present with pigmented skin findings. For example, disseminated histoplasmosis or cryptococcosis can rarely cause brownish papules or nodules, and Kaposi sarcoma may initially appear as violaceous or reddish-brown macules before progressing to plaques or tumors.
It is critical to emphasize that no single skin lesion is pathognomonic for HIV infection. Diagnosis relies on laboratory confirmation via antigen/antibody immunoassays, nucleic acid testing (NAT), or rapid point-of-care tests—never clinical appearance alone. Anyone experiencing new or unexplained skin changes alongside systemic symptoms such as persistent fever, night sweats, unintentional weight loss, or recurrent infections should seek prompt medical evaluation and HIV testing, especially if risk factors for exposure exist.
Early diagnosis and initiation of antiretroviral therapy (ART) remain the cornerstone of HIV management—preserving immune function, preventing progression to AIDS, and reducing transmission risk. Dermatologic evaluation may be warranted to identify and treat underlying causes of pigmentary changes, but these findings should always be interpreted within the broader clinical context.
