Dental professionals emphasize that thorough preoperative assessment—including panoramic radiography, complete blood count, coagulation profile, and cardiovascular evaluation—is essential before any extraction. Equally critical, yet often overlooked by patients, is the management of injection-related anxiety. To enhance both safety and patient comfort, clinicians routinely employ topical anesthetic agents prior to local anesthetic infiltration—a step known as pre-injection anesthesia.
The use of pre-injection anesthetics serves multiple evidence-based purposes: mitigating procedural anxiety, reducing anticipatory pain, suppressing salivary and bronchial gland secretions, blunting vagal reflexes, and lowering the incidence of postoperative nausea and vomiting. These agents are not merely adjuncts but integral components of modern, patient-centered oral surgery protocols.
In clinical practice, ideal pre-injection anesthetics must demonstrate rapid onset, effective mucosal penetration, and a favorable safety profile. One widely studied and prescribed option is compound chamomile-lidocaine gel (brand name: Ganmeida®), a prescription-only topical formulation containing lidocaine hydrochloride, chamomile tincture, and thymol. Lidocaine, the primary active ingredient, is a potent amide-type local anesthetic with high lipid solubility, enabling efficient diffusion across oral mucosa.
