Can HIV be transmitted through the saliva of a person with HIV?
No, HIV cannot be transmitted through saliva under normal circumstances. The concentration of HIV in saliva is extremely low—so low that it is considered non-infectious. Saliva contains natural antiviral proteins, such as secretory leukocyte protease inhibitor (SLPI) and defensins, which inhibit HIV replication and reduce viral infectivity. Extensive epidemiological studies have never documented a single case of HIV transmission via kissing, sharing utensils, drinking from the same cup, or other casual contact involving saliva.
Transmission requires direct access of infectious bodily fluids—such as blood, semen, vaginal fluids, rectal fluids, or breast milk—into the bloodstream or across mucous membranes. While theoretically possible in highly unusual scenarios (e.g., deep, traumatic oral contact with visible, active bleeding in both individuals), such cases are not supported by clinical evidence and are not considered realistic routes of transmission.
It is important to emphasize that HIV is not spread through air, water, food, insects, or casual physical contact—including hugging, shaking hands, or sharing toilets.
